   Date of Loss: 05-17-01
   Date Filed: 09-25-01
   Allegation: Failure to investigate a felony
3. Claimant: Jacobus Baas
   Date of Loss: 04-28-01
   Date Filed: 09-26-01
   Allegation: Trip and fall
